Better story than the last one (less fluffing around to get you to learn the basics)
Better weapon systems (can pick up bullet shells and repack them with stronger materials to make better ammunition, weapons specific vats commands, etc...)
karma plays more heavily into how the story gets fleshed out
Clothing now affects wether you get rushed by gangs (can be used to your advantage ;))
